Paracetamol is safe to use during pregnancy.

For the treatment of non-exacerbated rheumatoid arthritis, its use in pregnancy is considered to be safe by the American College of Rheumatology.
However, paracetamol should be avoided if the woman is pregnant or breast-feeding. In this case, NSAIDs or COX-2 inhibitors are preferred.
If paracetamol is used for the treatment of fever in a pregnant woman, its administration should be discontinued as soon as possible, to avoid its absorption by the fetus.
Caution in infants
In infants, paracetamol should be used with caution due to the risk of excessive thermal development, also known as hyperthermia, which can result in serious injury or death.
Death due to excessive hyperthermia is very rare in infants. It is uncommon in children and rare in adults. Serious injury has been reported, but most of the injuries were due to the administration of a very high dose of paracetamol to treat fever.
A high dose of paracetamol in a small child can cause a rapid, severe rise in body temperature. Infants are more sensitive to the side effects of paracetamol.
The U.S. Food and Drug Administration (FDA) has stated that safety and effectiveness of paracetamol have not been established for the treatment of infants younger than 2 months, in comparison to other non-steroidal anti-inflammatory drugs (NSAIDs).
It is recommended that an NSAID be used to treat fever in infants.
